Texas Health Seay Behavioral Health Center Allen is seeking to hire a Social Worker to work PRN (as needed) variable hours.

We are currently looking for PRN coverage for our Allen Behavioral Health inpatient and outpatient facilities.

DAYS & HOURS: Variable; days, nights, weekdays and weekends as needed.


Master’s Degree in Social Work or Behavioral Science


Two years psychiatric experience


LMSW and AHA BCL current. CPI within 30 days of hire.


Case Management and Therapy - 55%

Involves multidisciplinary team when appropriate. Consistently completes psychosocial, treatment plans and ensures they are signed and on the chart in 72 hours as well as ensuring the treatment plans are individualized. Consistently completes a minimum of 6 groups a week and documents them within 24 hours. Less than 3 family complaints annually. Flexible with patient assignments. Discharge plans are discussed with family, doctor and multidisciplinary team 48 hours prior to discharge.

Compliance in all areas of timely documentation. Volunteers to help other staff when needed. Families rate positive experience. Discharge plans are completed and appointment day and time set prior to discharge. Anticipates families' and patients' needs and responds. Serves as a resource/role model for other therapists. Cooperative, dynamic, more diverse with difficult patient assignments.

Provides direct patient care and coordinates all patient care provided by the multidisciplinary team.

Produces documentation that evaluates outcomes of patient care. Uses CPM philosophy to obtain adequate data to initiate, implement and revise plan of care to meet patient, physician and family needs. - 25%

Recognizes / initiates multidisciplinary referrals. Carries out therapy timely. Anticipates potential complications, intervenes appropriately. Consistently reassesses patients in timely manner. Independently gathers and interprets information related to disease process/diagnosis. All elements of the therapy process are complete 95% of the time. Care planning is individualized. Patient teaching and discharge needs met prior to discharge. Recognizes/initiates multidisciplinary referrals. Anticipates potential complications, intervenes appropriately. Documentation reflects consistent reassessment of patient needs and updates of the plan of care. Meets standards of patient care. Recognizes changes in patient condition and intervenes early to prevent / lessen complications, documents accordingly.

Prepare patient to be discharged on time. Efficiently implements discharge plans. Serves as a resource /role model. Meets, and exceeds the standard of care. Consistently evaluates patient care outcome. Serves as resource in recognizing potential complications and takes action proactively. Volunteers to complete progress round documentation for others when they are busy. Cooperative, dynamic, more diverse with difficult patients and assignments. Offers to do more than their fair share of the workload. Serves as a Mentor/resource to staff for proactively incorporating the multidisciplinary approach in the patients care processes. Coaches the team to anticipate discharge needs and over come barriers to patients discharge. Is proactive in addressing potential patient needs/issues.

Achieves professional growth and development through participation in educational programs, providing inservices, actively participates in staff meetings, mandatory, committee meetings. - 10%

Attends / participates 10 staff meetings and is consistently on time. Shares information. Attends all mandatory inservices. Assists with skills day. Completes all assigned projects and staff education in a timely manner. Licensure and certification renewals completed in a timely manner. Turns in documentation early without prompting. Willingly accepts student and seeks opportunity for interaction. Fosters good experience. Provides input for improvement to unit based orientation.

Member of professional organization or certifications pertinent to area.

Presents in-services, actively participates in committees, process improvement projects and special assignments, coaches others to do the same. Participates a housewide or system wide committee. Attends 2 workshops pertinent to specialty and gives 2 in-services (1 formal and 1 informal) per year. Is a driving force behind the mission/vision of the partnership council. Specialty certifications. Provides input for improvement to unit based orientation. Facilitates and incorporates improvements into orientation process. Ensures a conducive learning environment for orientees.

Promptly answers telephone and promptly returns calls. Relays messages accurately and in a timely manner. Identifies self and department when answering phone. Participates in community service projects at THP sponsored activities. Will assist lost visitors, provide tours, discuss program with potential patients and families. Redirects employees exhibiting unprofessional behavior, and consistently maintains professional behavior. Safeguards personal information about others. Is aware of safety issues and corrects them. Identifies needs and responds quickly and positively to requests. Questions anyone not wearing proper ID. Asks, "What is best for the patient". Acts as a patient and family advocate. Wears ID.

Volunteers to answer phones, calls patient's family prior to family calling. Consistently maintains professional atmosphere on unit consistently. Maintains and encourages a professional attitude for employees on all units. Role model for service excellence. Coaches others to prevent breach in confidentiality. Seeks and corrects safety issues, coaches others to make corrections. Anticipates needs and seeks opportunities to assist others. Seeks opportunities to provide comfort and care, putting patients first and is a role model for the team.

Has had an occasional missed punch, absence, or tardy but has not received any verbal or written warnings.

Consistently follow department schedules, guidelines. Anticipates problems with the schedule and is flexible to unit needs.

Adheres to hospital and unit guidelines for breaks, personal calls, cell phone and computer use. Uses hospital resources appropriately, including human resources, time and supply resources.
