Texas Health HEB seeks to hire a full time RN for the Outpatient Wound Clinic. This opportunity will be working PRN when needed

The ideal candidate will possess the following qualifications:

BSN preferred.

*RNs hired after January, 2014, require completion of BSN within 3 years of start date.

**RNs hired within THR after December, 2016, require completion of BSN within 2 years of start date.

One (1) year experience as a clinical nurse, or completion of an RN residency program, or acceptance into a THR residency program.

Current RN licensure or compact licensure recognized by the Texas Board of Nursing with current AHA BCLS upon hire.

A high degree of confidentiality, positive interpersonal skills, and ability to function in a fast-paced environment.

One (1) year wound care experience preferred but not required.

CWON within 6 months of hire.

Job Responsibilities:

Nursing Process: Assessment and documentation related to patient care duties and responsibilities.

  • Consistently completes a comprehensive admission history, including the medication reconciliation process (if applicable for the unit of employment).

  • Consistently performs risk assessments and implements interventions according to protocols; monitors and updates interventions as needed (ie. age specific, fall, skin, isolation).

  • Initiates and updates the plan of care and education record using established CPM guidelines.

  • Consistently exercises good judgment in setting priorities related to care of the patient

  • Comprehensive pain assessments are performed, re-assessed, and documented; actions are taken as necessary.

  • Consistently demonstrates timely, complete charting with re-assessments during shift; documentation is accurate and legible.

  • Assessments and interventions are based upon, and demonstrate best practice / EBP (evidenced based practice).

  • Consistently documents required elements to meet Core Measure and KPI requirements.

  • Able to anticipate complications based on clinical assessment (including physical exam and diagnostic tests) and uses critical thinking skills and knowledge to pro-actively intervene.

  • Performs rapid observation and assessment and interventions to improve patient outcomes.

  • Coordinates and utilizes resources effectively in response to changing clinical status and emergent situations.

Maintains and demonstrates competency in BLS (Basic Life Support), infections control, safety and all unit required skill review.

Teamwork and communication- the ability to work effectively and productively with all members of the patient care team, including other disciplines, physicians, and family members / significant others.

  • Facilitates the development of a trusting relationship with all customers.

  • Willingly takes initiative in assisting others.

  • Responds to patient needs/call lights promptly.

  • Consistently utilizes proper phone communication skills.

  • Demonstrates adaptability and flexibility to changes in schedule and / or workload assignments necessary to meet changing needs of the individual patient or unit.

  • Identifies opportunities for, assists with implementing, and supports change
