Supervisor - Community Health

Work location: Mostly remote, however, may need to come into the office for meetings and/or training

Work hours: Full-time, Monday through Friday from 9:00 am to 5:00 pm, however, there may be evening or weekend hours

Here’s What You Need


Associate's Degree Public Health / Health Care Related Studies 2 Years REQUIRED or

Bachelor's Degree Public Health / Health Care Related Studies 2 Years Preferred and

Other CHW Instructor 2 Years Qualified to instruct CHW certification courses and continuing education Preferred


2 Years of experience working with clients in the community to connect them with resources and support clients to find and engage with resources REQUIRED and

1 Year Experience leading and managing people Preferred

Licenses and Certifications

CHW Certification Upon Hire REQUIRED


Strong verbal and written communication skills

Ability to maintain composure in stressful situations

Strong customer service skills

Ability to problem solve

Time management skills

Detail-oriented with strong organizational and prioritization skills

Ability to work independently as well as with others

Ability to motivate and inspire others

Ability to complete required reports and documentation

What You Will Do

Supervises daily activities of CHWs to ensue efficient operations, including

a. Interviewing, training, and performance evaluation

b. Oversees staffing and provides coverage when needed

c. Performs administrative duties related to payroll management

d. Effectively mentors community health worker team

e. Advocating for staff needs in the entity clinic and community setting

f. Works collaboratively with CHI program leaders to ensure CHW team supports programs in assigned region/entity service area

Manages community health worker staff

a. Holds team members accountable for duties as assigned

b. Provides necessary training and orientation to CHI programs and processes in support of a place-based approach to CHW support

c. Ensures integrated approach to client care across CHI programs and regions is maintained

d. Ensures CHW team stays up-to-date on all Texas Health training requirements through MyTalent

e. Stays current on CHW professional practice and ensures team receives inservice training and updates

f. Addresses corrective action appropriately and in a timely manner

g. Responsible for regular meeting cadence for team to support communication, knowledge sharing and troubleshooting

h. Is available to CHW personnel during team work hours to answer questions and provide guidance as needed

Maintains outcomes data and dashboard to allow for program evaluation.

a. Collaborates with CHI program leaders to understand and support data collection needs from the CHW role

b. Reports data at determined intervals to meet dashboard reporting needs

c. Shares data outcomes with team and works collaboratively to adjust practice based on findings

Supports customer satisfaction by

a. Addressing and working to resolve client and community partner complaint

b. Learning and understanding the ethnicity, language, socio-economic status and life experiences of the communities served

c. Maintains knowledge of and supports staff use of reliability and safety tools

d. Ensures staff are trained in customer satisfaction principles and tools

Maintains skills in direct client work including

a. Teaching skills

b. Interpersonal skills that encourage relationship building with a diverse group

c. Strong communication skills, both verbally and in writing

d. Knowledge of Texas Health and community resources available to meet client needs

e. Motivational interviewing skills

f. Knowledge and understanding of all CHI programs and makes appropriate referrals

g. Flexes into role of front line CHW as needed

Supports and maintains collaborative work environment

a. Fosters interdepartmental communication focusing on operations and logistics of the place based CHW strategy

b. Collaborates with CHI leadership and ensures that collaborative work environment is maintained

c. Collaborates with entity clinic leadership

d. Communicates with entity leadership in collaboration with Sr. Director

e. Works collaboratively with community partners and develops relationships that expand the reach of the program

f. Supports capacity building and advocacy skills
