Texas Health Resources is seeking a Community Health Worker PRN.
Locations and Hours:
Position will primarily work in Dallas/Denton Area
PRN “As Needed”
Minimum - $17.61
Maximum - $24.17
Please note salary is determined by the number of years of experience
The Community Health Worker (CHW) will serve as a community liaison supporting the Community Health Improvement strategy for Texas Health Resources. The CHW will be responsible for group and individual classes around disease management and lifestyle wellness. The CHW will spend part of their time supporting a local Texas Health provider which will require providing support for patients as part of a care coordination plan. The CHW will connect patients with community resources to address social determinants of health (barriers to care) and will help patients navigate the health system as appropriate. Home visits may be required for complex patient needs.
Coordinate Community Health Improvement Activities
Participate in program development and planning
Develop and effectively manage program timelines to ensure timely completion of program deliverables
Monitor program activities and reporting to ensure accuracy in outcome measures
Facilitate small- and large-group education
Establish and manage relationships with community members, organizations and coalitions
Identify strategic partnership opportunities and promote relationship-building
Disseminate programmatic and marketing materials
Develop presentations and other materials for use during program-related events
Serve as programmatic liaison to internal and external stakeholders
Responsible for establishing trusting relationships with patients and families
Provides ongoing follow up via phone calls or home visits as appropriate
Navigates patients and families through medical and non-medical support services
Conducts eligibility, enrollment and follow up for referred medical or non-medical support
Helps patients set personal care plans and attend appointments
Works closely with medical care team to ensure coordinated care
Serve as liaison to internal and external resources through TH network
Report activites through CBISA
High School Diploma
Bachelor's Degree preferred
Successful completion of a Community Health Worker formal training program such as from a community college or other education institution preferred
- One year experience Coordinating volunteer services/programs, public speaking, completing activity reports, recruiting volunteers, and partnering with external organizations.
