Texas Health Resources is seeking a Community Health Worker PRN.

Locations and Hours:

  • Position will primarily work in Kaufman/Johnson/Erath Area

  • PRN “As Needed”

Salary:

  • Minimum - $17.61

  • Maximum - $24.17

  • Please note salary is determined by the number of years of experience

Position Summary:

The Community Health Worker (CHW) will serve as a community liaison supporting the Community Health Improvement strategy for Texas Health Resources. The CHW will be responsible for group and individual classes around disease management and lifestyle wellness. The CHW will spend part of their time supporting a local Texas Health provider which will require providing support for patients as part of a care coordination plan. The CHW will connect patients with community resources to address social determinants of health (barriers to care) and will help patients navigate the health system as appropriate. Home visits may be required for complex patient needs.

Job Responsibilities:

Coordinate Community Health Improvement Activities

  • Participate in program development and planning

  • Develop and effectively manage program timelines to ensure timely completion of program deliverables

  • Monitor program activities and reporting to ensure accuracy in outcome measures

  • Facilitate small- and large-group education

  • Establish and manage relationships with community members, organizations and coalitions

  • Identify strategic partnership opportunities and promote relationship-building

  • Disseminate programmatic and marketing materials

  • Develop presentations and other materials for use during program-related events

  • Serve as programmatic liaison to internal and external stakeholders

Patient Support

  • Responsible for establishing trusting relationships with patients and families

  • Provides ongoing follow up via phone calls or home visits as appropriate

  • Navigates patients and families through medical and non-medical support services

  • Conducts eligibility, enrollment and follow up for referred medical or non-medical support

  • Helps patients set personal care plans and attend appointments

  • Works closely with medical care team to ensure coordinated care

  • Serve as liaison to internal and external resources through TH network

Qualifications

Education:

  • High School Diploma

  • Bachelor's Degree preferred

  • Successful completion of a Community Health Worker formal training program such as from a community college or other education institution preferred

Experience:

  • One year experience Coordinating volunteer services/programs, public speaking, completing activity reports, recruiting volunteers, and partnering with external organizations.

Skills and Abilities:

  • Strong verbal and written communication skills

  • Ability to maintain composure in stressful situations, ability to problem solve, time management skills, detail-oriented

  • Ability to work independently as well as with others

  • Strong organizational & prioritization skills

  • Ability to motivate and inspire others

  • Ability to complete required reports
