Texas Health Resource is seeking to hire a Supervisor Post-Acute Clinical Registered Nurse to support our Southwest Health Resources Utilization Department.

Salary range is $37.62/hour – Max $53.03/hour – based on relevant experience.
• Full-time: 40 hours per week, Monday-Friday
Under the direction of the PHSC Post-Acute Manager, the PA Clinical Supervisor ("Supervisor") is responsible for accelerating the delivery of high-performing clinical care, case management of SNF episodes, and program development while performing
daily administrative oversight of program staff. In addition, the role participates in improving the "PHSC Aligned Post-Acute Network" post-acute provider performance, including the adoption of evidence-based clinical practice through collaborative
staff education to ensure reliable quality and delivery of services. The Supervisor is responsible for improving care delivery, managing the post-acute episode of care (reduce unnecessary LOS and unplanned readmissions) in order to decrease cost and improve patient outcomes and experience for ACO beneficiaries.
Experience discerning the variation and complexity inherent in caring for vulnerable populations, including a capacity to evaluate and optimize available healthcare resources is an essential. The essential job duties for this position are:
• Consistently ensure individual, departmental, and team excellence through daily supervision of and workforce management.
• Conduct quantitative and qualitative analyses on program metrics to effectively report program impact to continually demonstrate value.
• Participates in internal and external committees.
• Formulate strategic plan and coordinate goals with management.
• Supports a variety of projects and programs in the PHSC portfolio.
• Proactively identify and mitigate risk by responding to complaints or concerns from clients, staff and/or program participants. Concerns may include HIPAA, compliance, background checks, policy violations and more.
• Performs all functions of the PA Clinical Specialist role.
• Clinical educator for enhanced skills competency and disease and symptom management of ACO beneficiaries.
• Train all incoming program staff using standardized curriculum materials focused on patient care, professionalism, communication and compliance.
• Provide staff mentorship, coaching and professional development opportunities including professional development workshops.
• Implement patient health education programming in one-on-one and group settings to increase the knowledge and self efficacy to prevent illness, manage health conditions, and improve overall wellness.
• Serve as a member of an inter-disciplinary clinic, care team, and promote effective working relationships with internal and External stakeholders.
• Develop strong relations with academic and community organizations to keep abreast of industry best practices.
The ideal candidate will possess the following qualifications :
• Associate’s degree nursing required. (Associate degree with 5 years leadership and 7 years’ experience in lieu of Bachelor’s may be considered).
• Bachelor’s nursing or Master’s degree MSN, MBA, MEd preferred.
• 3 years’ experience demonstrated successful leadership of clinical teams and or population health programs required and 5 year Registered Nurse in an acute clinical care setting, care management with direct patient encounters, or patient triage or 7 years skilled nursing experience required an Emergency Department, Intensive care Unit or Progressive Care Unit preferred.

• Ability to manage multiple tasks and competing priorities.
• Strong organizational skills.
• Capable of managing highly confidential information.
• Demonstrate proficiency in computer keyboard skills and use of common office applications including MS Office Suite.
• Broad understanding of level of care environments including acute hospitals, post-acute facilities, home health, DME,
outpatient services, and PCO clinic operations.
• Ability to track and trend data.
• Demonstrate professional flexibility with scheduling, travel and competing priorities
