Texas Health Resources seeks to hire a Social Worker to work as a Post Acute Patient Care Manager in our ACO Utilization Management department. This position is primarily field based, working in post- acute facilities directly with patients and families.
40 hours weekly, Days
Monday - Friday (8am-5pm) some flexibility needed
Approximately 50% local travel required.
Bachelor's Degree in Social Work required
Master's Degree in Social Word preferred
3 years' working in a community / outpatient setting, in the geriatric field working in a direct service capacity, or social work experience in an inpatient setting. required.
3 years' clinical experience in post- acute setting preferred.
LICENSE / CERTIFICATION:
Required Upon Hire:
LBSW - Licensed Bachelor Social Worker or
LMSW - Licensed Master's Social Worker and
DL - Driver's License - Must be valid
