Ostomy Care/Health Education Lifestyles Program
Texas Health Physicians Group is pleased to announce a Nurse Practitioner opening for an outpatient program that is designed to help uninsured individuals with chronic disease management and ostomy care by encouraging lifestyle changes and providing access to affordable care.
Monday –Friday Full Time Schedule
Assists patients in managing chronic conditions such as diabetes, high cholesterol, and hypertension.
Participate in ongoing health coaching and provide education resources to support patients learning to effectively manage their chronic disease.
Ostomy care for patients with acute and chronic wounds
EPIC EMR documentation system
Comprehensive benefit package including health, life, dental, vision CME, Malpractice and 401k
Graduate of accredited Nurse Practitioner program
NP Certification and TX license required
Experience in Ostomy care required
The Nurse Practitioner (“NP”) Must be a graduate of an accredited Advanced Nursing Program and have a current RN license for the State of Texas.
Certified Advanced Nurse Practitioner in the State of Texas.
The NP will report to the supervising physician and/or the Medical Director and will be responsible for meeting CME requirements to maintain certification and licensure.
The NP’s duties and responsibilities apply to the THPG patient population and assigned setting only.
